Dual Enrollment Information/Interest Survey

If you are interested in taking dual enrollment courses, please fill out the attached survey.  Please review the information below carefully to ensure eligibility.  


Steps for Dual Enrollment

Meet the eligibility requirements

Earned at least 6 high school credits
Have at least a 3.0 unweighted GPA
Take placement test (PERT, SAT or ACT)

Fill out and submit the dual enrollment interest survey.

Complete the online application www.polk.edu

Once you submit your application, you should receive within two business days an email from Polk State with general information and your Polk State ID number.

You must bring your ID number to PERT testing.

The PERT consist of three sections; Reading, Writing and Mathematics.  Each section has 30 questions and the test is untimed.  Scores are given once the test is submitted.

Passing scores:  PERT – Reading 106, Writing 103, Math 114

                             ACT – Reading 19, Writing 17, Math 19

                             SAT – Reading 24, Writing 25, Math 24

Meet with Mr. Haun after testing to see what course/s you are eligible to register for and enter into DELTA

Proof of Orientation must be shown on day of registration.

Register for course/s with a Polk State Advisor.
